WHAT TO EXPECT ON YOUR FIRST VISIT
Once you and your doctor decide to use MD Home Health services, our staff will visit your home to assess your needs and medical condition. We will collaborate with you and your doctor to create a comprehensive care plan, including all necessary services, treatments, and equipment.
Our team will make regular visits to your home to implement the care plan, which will be reviewed regularly by our staff and your doctor. If your health condition changes, we will coordinate with you and your doctor to adjust your care plan as needed. During our visits, we will educate you and your caregivers on how to continue any necessary care, such as wound care, therapy, and disease management. We will also teach you how to recognize potential problems and side effects, and whom to contact if they occur.
When you have met your established goals, your doctor feels it is appropriate, and/or you are no longer eligible, your home health services will be discharged. Your doctor may recommend continuing treatment at an outpatient facility or scheduling regular follow-up appointments.
